Is Independent Medical Humanitarian Assistance Possible?

In some of today´s conflicts we see an increasing brutalism where attacks on hospitals and clinics become a dangerous new norm. Humanitarian aid seems to be co-opted as part political and military strategies. How did we end up undermining the respect of humanitarian medical action?
